CONFERENCE REGISTRATION

Optional Activity and attendees must register in advance.
If the names listed for the foursome are different from the names listed for the conference registration the NYSAC office will be aware that you are indicating the names of the requested foursome, not registering additional attendees. NYSAC will ensure to the best of it's ability that all golf pairing requests are provided.

Optional and in addition to the conference registration fees stated above.

Exhibitor table set-up time begins at 12:00 p.m. on Sunday, September 15, 2019.

SPONSORSHIP REGISTRATION

Optional. If registering to be a sponsor, please send your cemetery/business logo to the NYSAC office at the following email address: shm@fwc-law.com. This logo will be used on the printed materials and electronically throughout the event. If you do not have a logo, NYSAC does have alternative options available. Please contact the office if you are interested in being a sponsor, but do not have a logo.
*Platinum PLUS includes exclusive sponsorship of a coffee break. **Gold PLUS includes shared sponsorship of the Annual Banquet Cocktail Hour.

MEMORIAL SERVICE OF REMEMBRANCE

Optional. Please submit the names of those individuals to be included in the memorial service on Monday, September 16, 2019. In addition to the names, if you have a picture and obituary please send this information to the following email address: shm@fwc-law.com.
The Memorial Service of Remembrance will take place on Monday, September 16, 2019 at 8:30 a.m during the Annual Members Meeting.

To all members of the New York State Association of Cemeteries:

Notice is hereby given that the Annual Meeting of the New York State Association of Cemeteries will be held on Monday, September 16, 2019 at 8:30 a.m. at the 2019 Fall Conference at The Thayer Hotel in West Point, NY..

* Please Note: If the registered member cannot attend this meeting, please fill out proxy enclosed in the registration form at the following link: CONFERENCE REGISTRATION FORM. Members can also call the NYSAC Office at 518-434-1134 to receive a proxy. The proxy should be completed and returned to the NYSAC Office via fax (518-427-0452) or mail by Friday, September 6, 2019.

REPORT FROM THE NOMINATING COMMITTEE

The following have been nominated as Officers of the New York State Association of Cemeteries for the term September 2019 to September 2020:

President Philip J. Tassi – The Ferncliff Cemetery Association, Inc.

Vice-President Jeffrey Reed – Mount Calvary Cemetery Group

Secretary - Nathan J. Romagnola -White Haven Memorial Parks, Inc.

Treasurer Richard J. Moylan – The Green-Wood Cemetery

The following have been nominated to the Board of Directors of the New York State Association of Cemeteries for a three-year term ending September 2022:

Michael Austin - Elmlawn Cemetery

Domenick Castiello - Mount Hope Cemetery

Joseph Dispenza - Forest Lawn Cemetery

- Registration must be done by Friday, September 16, 2019 to allow NYSAC to be able to accommodate all attendees.

- Registration will not be refunded unless NYSAC is notified 3 days prior to the conference.

- Please send hotel reservation and hotel payment to The Thayer Hotel , not to the NYSAC Office. Hotel reservations can be made by calling The Thayer Reservations Department at 845-446-4731 ext. 7997 and advising them that you are with the New York State Association of Cemeteries
- Hotel reservation cut-off date is Thursday, August 15, 2019.*
